• Hospitalizations: Date, Dx, Tx
• Immunizations, drug sensitivities, allergies
• Med compliance

Alcoholoism screening

A	Ever get Annoyed by criticism of drinking?

Components of Family Hx

• Age & state of health of parents, siblings, spouse, and children
• Hx of tuberculosis, DM, htn, heart attack, stroke, arthritis, cancer, alcohol, drug use, emotional problems
• Hereditary disease, unusual illnesses, early deaths
• Effects of patient health on family and vice-versa; "taboo" diseases like HIV

A	Activities in a typical day: diet, exercise, hobbies, sleep habits
D	Drug use: illicit
S	Sexual hx, suicidal or depressed (ever), spiritual hx

Illicit drugs
